			<p>The <strong>Personal History Survey (PHS)</strong> is a web-based, branching survey instrument designed to gather information relevant to a respondent&rsquo;s psychological suitability for employment in a public safety position. The <strong>PHS Report</strong> is generated from a respondent&rsquo;s completed PHS and is a narrative-form report intended for use with police, correctional, dispatcher, firefighter/medic, security, and other public safety, public trust, and safety-sensitive positions. The PHS can be used as a standalone instrument or alongside the MMPI<sup>&reg;</sup> and MPQ<sup>&reg;</sup> as part of a comprehensive pre-employment psychological evaluation.&nbsp;</p>

			<p>The PHS can be used:&nbsp;</p>

			<ul>
				<li>As part of a standard assessment battery for conducting pre-employment psychological evaluations of public safety applicants or candidates&nbsp;&nbsp;</li>
				<li>To guide the review of personal history information and the postoffer clinical interview&nbsp;</li>
				<li>By background investigators (preoffer version) to aid in organizing and exploring job-relevant, nonmedical personal history&nbsp;&nbsp;</li>
			</ul>

			<h3>Key Features&nbsp;</h3>

				<li><strong>Flexible domains and questions&nbsp;</strong>The PHS allows users to deselect various domains and individual questions to create a custom administration. Users can also choose from preconfigured setups that include preoffer option for public safety applicants or a post-offer option for public safety candidates.&nbsp;</li>
				<li><strong>Streamlined administration&nbsp;</strong>This web-based, branching survey instrument simplifies the assessment process by administering additional questions based on the individual&rsquo;s initial responses.&nbsp;</li>
				<li><strong>Detailed background information on the respondent&nbsp;</strong>PHS questions foster candour and accuracy, providing greater specificity and detail than competitive products. The survey collects personal history information in up to 17 domains: Adverse Childhood Experiences, Biased Attitudes and Behavior, Childhood/Adolescent Relationships, Demographics, Driving, Education, Employment, Family of Origin, Finances, Gambling and Gaming, Legal, Life Events, Marital/Partner/Child Relationships, Military Service, Physical and Mental Health, National Security Clearance, Sexual Behavior, and Substance Use.&nbsp;&nbsp;&nbsp;</li>
				<li><strong>Includes positive and negative history&nbsp;</strong>The PHS supports psychological screening focused on risks of counterproductive behaviors and negative outcomes and on traits and characteristics that may mitigate risk and contribute positively to work performance.&nbsp;</li>
				<li><strong>Narrative statements based on the respondent&rsquo;s self-reported history&nbsp;</strong>The Q-global generated report PDF reproduces the respondent&rsquo;s self-reported history in narrative form, organized by domain. Statements can be copied from the PDF and used for independent report writing&mdash;resulting in significant time savings.&nbsp;</li>
				<li><strong>Informed by research&nbsp;</strong>The PHS questions are derived from empirical findings in public safety assessment and specifically target risk factors relevant to both preoffer applicants and postoffer candidates.&nbsp;</li>

			<p>The PHS Report can be used for the assessment of personnel in the following public safety sectors:&nbsp;</p>

			<strong>Correctional and probation</strong> &ndash; correctional officers, parole agents, juvenile detention counselors<br />

			<strong>Private security</strong> &ndash; armed and unarmed guards, executive protection agents, campus and event security<br />
			<strong>National security and defense</strong> &ndash; TSA officers, border agents, intelligence staff, military personnel in sensitive assignments<br />
			<strong>Critical infrastructure</strong> &ndash; nuclear reactor operators, utility security personnel, armored transport crews<br />
